Transition Plan

During the past month one of my students turned 30 months old, so I created a Transition Plan to help the child go from the Home-Based setting to a CAP center. During transition, I am able to review the child's development with the parents. This is always a home visit that the parents and P.E. enjoy. C's Mom was so excited to see how much C had learned in all developmental areas such as - language, motor and social skills. During my visit C's Dad came home from work, Mom was so excited to tell him all about C's progress and she encouraged Dad to set new goals with her for C. C's Parents were so grateful to the EHS HB program and are determined to continue setting new goals for themselves and their family.
